Submitting a request for examination to Health Ministry
A person wishing to submit a claim for a mobility benefit to the National Insurance Institute must submit an Application Form for Medical Examination to Establish Mobility Disability to the District Health Bureau.
Claim processing at the National Insurance
After the district medical committee or the Ministry of Health's medical appeals committee establishes the percentage of mobility disability, entitling you to a benefit, the National Insurance will receive the medical protocol and consider it as a claim for mobility benefits.
Please note, if needed, the insured will receive a letter asking for additional documents, to enable us to continue processing the claim.
The documents must be submitted to your local NII branch closest to your place of residence. You can submit the documents by mail, by fax or in the service box of the local branch, or through the document delivery service on the website.
Documents to provide to complete the processing of mobility benefits claim:
In a claim for a mobility allowance for vehicle owners:
A photocopy of a valid driver's license with details of all driving restrictions.
A photocopy of a valid and comprehensive insurance policy.
If the person with limited mobility is an earnerA person whose income from work is more than 25% of the basic amount - NIS 2,403 (as of Jan 01, 2023) NIS 2,585 (as of Jan 01, 2024) ,as well as a soldier in compulsory army service. (70), he must also attach pay slips for the three months preceding the date of submission of the claim.
A soldier in compulsory service must submit certification from the army regarding his service and estimated date of release.

In a claim for a mobility allowance for persons without a vehicle:
If the person with limited mobility is an earner, he must attach pay slips for the three months preceding the date of submission of the claim.
A person who lives in an institution must attach certification of his departures (in a vehicle) from the institution - at least six departures a month, including details of the departure times.

Travel expense reimbursement
If in order to come to a Medical Board or Appeals Medical Board, you need to travel and are unable to use public transportation because of your medical condition - you may be entitled to reimbursement of your travel expenses, on condition that you do not receive a mobility allowance.
For the purpose of examining your entitlement to a refund, you must provide your local NII branch with authorization from the district physician or the chairman of the appeals committee, stating that you are unable to use public transportation, along with original receipts from the trip.
